Download the Survivor application from the CBS casting website and print it out.
Read the eligibility requirements to ensure you are eligible to apply for Survivor. In particular, applicants must be at least 18 years of age at the time of the application. In addition, applicants who reside in Alabama, Nebraska and Wyoming must be at least 19, and applicants who reside in the District of Columbia or Mississippi must be at least 21 years old.
Answer all the questions on the Survivor application, including signing and dating at the bottom. This is your chance to show the producers of the show what you are like and why they should select you as a semi-finalist.
Read the Release and Waiver form and sign and date it at the bottom.
Film a video of yourself that addresses the topic of "Who you are and why you would make the ultimate Survivor." The video must be three minutes or less in length and must be submitted on a VHS tape or DVD with your name clearly written on it, along with the number of the city nearest you, as outlined in the application instructions. If somebody helped you film the video, they must read and sign the Videographer Release Form included in the application packet.
Obtain a passport-size photo of yourself and include it with your Survivor application.
Photocopy your valid United States passport and include it with your application. If you do not have a passport or it has expired, you must apply for a passport and include a photocopy of the form you receive as proof that you have applied for a passport.
Mail all parts of your Survivor application in a single envelope to: Survivor/2801 Ocean Park Blvd./Santa Monica, CA 90405. Also write on the front of the envelope the number of the city nearest you, as outlined in the application instructions. The application must be mailed by the deadline specified in your application packet.
