Fill out the information requested on this page. This includes your name, email address, operating system, iTunes version, age and the type of your request.
Enter detailed information about your request into the "Comments" box near the bottom of the page. This is where you can specify the exact artist, show, song, movie, album or other item you wish the iTunes store to carry.
Click the blue "Submit Feedback" button when you are done. This submits your request.
