Download and print the Extreme Makeover Home Edition application from the ABC website. You can find the link in the Resources section of this article. Include information about the family you are nominating. If you are not nominating your own family, you'll also have to include information about yourself. Additionally, explain why you feel the family is deserving of a home makeover.
Submit photos and a video of the family you are nominating and their home. You may send ABC a DVD or VHS. They do not however accept VHS-C or High 8 formats. Although it is highly recommended, a video is not required. However, you must submit photographs.
