If the editor's middle name is not given, omit the second initial. When there is more than one publication location, list the first one that appears on the title page or with the copyright information on the back of the title page.
For example, "The Cambridge Companion to the French Novel" was published in Cambridge, New York, and Melbourne. The properly formatted reference page entry for this work would be: Unwin, T. (1997). The Cambridge companion to the French novel. Cambridge: Cambridge University Press. (The title should appear in italics.)
