Is Jake in two and a half men the same kid since begging of show?

The character of Jake Harper in Two and a Half Men is played by two different child actors throughout the series. The youngest Jake was played by twin babies Sean and Sullivan Sweet from 2003 to 2004. These baby actors were uncredited. Following this point in the series, Jacob Justin Young played Jake from the years 2004-2015, until the shows conclusion.
