Download the appropriate application for your state's entertainment work permit. To find out if your state requires a work permit, visit childreninfilm.com and check out the rules and laws section. The dropdown menu will allow you to select your state and then link to the appropriate form.
Call the Children In Film permit service. You can reach permit services at 818-432-7400. Ask specifically for the permit service department and inform them you will need a rush work permit.
Complete the permit service application form. If your child is school age, you will need a signature from your child's school stating that he is in good standing with the school. This must be the original signature, not a copy.
Sign, as the parent, the work permit application form. Make sure you are signing the same form that contains the school's original signature.
Submit your permit application to the Children In Film permit service department. You can do this by mailing the permit overnight or bringing the permit to their offices in Studio City, California. Turnaround time is dependent upon how quickly your application is received by the permit service office. The fee will be $100.00 (As of August, 2010)for rush processing.
