Write your name either on a piece of lined paper or on the computer. Make it so that your first and last names are on two different lines, nine lines apart. Note that your first and last name will cradle or surrounded the other lines in your poem.
Write four traits that best describe you on the second line of your poem. These traits can include things such as creative, organized, attentive and understanding.
Write the words "Sibling of" and list the first names of any sibling you might have on the third line. Write "no one" if you're an only child.
Jot down a few things you like after the words "Who likes" on line four of your poem. Use descriptive words such as family, animals and books.
Write "Who feels" on line five of your poem followed by how you're feeling at the time. Include words such as happy, sad, lonely or thankful to your line of feelings.
Think about what you need and write your answer down on line six after the words, "Who needs." This can include just about anything from food, to love and family.
Write the words "Who gives" on line seven of your poem. Explain what you give or contribute to others, such as caring for animals as a volunteer at a local animal clinic.
Write the words "Who fears" on line eight and list a few of your fears. Note that these can be rational or irrational fears. Write down what comes to mind first.
Include the words "Who would like to see" on line nine and give an example of something you'd like to see at some time during your life. This can include things such as the Grand Canyon, Disney World or your next trip into space.
Write down your home address on line 10. Note that this includes your city, state and zip. Avoid listing your street address, unless you don't mind sharing it with other people.
