You unlock the 8th Island, Seven Island, in Pokemon FireRed by defeating the Elite Four and becoming the Pokemon Champion.
Once you've accomplished this feat, you'll be free to sail to the 8th Island and explore its unique areas, including the mysterious Seafoam Islands and the powerful trainers waiting to challenge you.
